How to Grow Your Practice with Great Marketing (and a Little AI)
This primary care practice was growing fast—two locations in Texas, a third on the way, and a phone that would not stop ringing. The front desk could not keep up with the call volume, and patients were the ones waiting.
The physician who founded and leads the practice, weighed the conventional answer—a 20-person call center—and decided it wasn’t the right fit, on both cost and the effort of managing it. He wanted a better option for his patients and his team. He found Talkie.ai.
Today, Talkie’s AI front desk assistant Jessica, answers, triages, schedules, and routes every incoming call around the clock, including after hours and weekends. The team only picks up the calls that actually need a person.

The practice did not have a phone problem because something was broken. It had a phone problem because things were going well.
The practice found Talkie at a pivotal moment. Growth had been astronomical and the phones were ringing off the hook. Staff simply could not keep up with the number of calls coming in, and the founder needed a solution.
The default answer—hire a call center—did not sit right with the founder.
Bringing on a call center staffed by 20 different people wasn’t the right move. The cost was one problem; managing an operation that size would have also been difficult on its own. Given the direction technology was heading, adopting something like Talkie was, to the founder, a great decision—and not a significant burden on the clinic’s financial performance.
Talkie’s team configured Jessica to the practice’s specific workflows before go-live, integrated her with the practice’s EHR, Elation Health, and ran a live virtual in-service so the whole team understood how the system worked from top to bottom. Patients got easy-to-understand informational materials explaining the new assistant, and a transfer to a human at the front desk is always one step away.
Jessica went into production on October 29, 2025, answering calls across both locations, including after hours and on weekends.
Ask the team what they appreciate the most about Talkie, and the answer is not “scheduling” or “answering common questions.” It is what happens before any of that.
They’ll say the most important thing Talkie does is triage the calls. By the time a call actually reaches the clinic, the team knows it has already passed through several layers—so they can be confident it’s something important that genuinely needs their time and attention.
The numbers bear that out. Talkie has answered nearly 22,000 calls in just over 8 months—handling the routine ones itself and steering anything it can’t resolve to the right person at the practice, already screened so staff don’t start each call from scratch. The front desk no longer works a flat queue of every call. They work a prioritized list of the calls that genuinely need them.
And the calls that come in after hours or on weekends now get answered instead of rolling to voicemail. Talkie takes the everyday requests itself and routes only genuine emergencies to the on-call line—so the on-call provider is no longer pulled away for a routine reschedule. The agent has captured and transcribed over 7,000 voicemails so far, so nothing falls through.
There was some early abrasion. Patients at one location were used to reaching a familiar front-desk voice directly. But adoption came faster than expected, even among the patients the team thought would resist.
Some of the practice’s senior patients were the most enthusiastic—several said they loved it, and they caught on quickly.
Without the EHR integration, day-to-day work would be extremely difficult—moving from one interface to another is time-consuming and takes a lot of clicks. Talkie integrates well with the practice’s current EMR, as well as the one they’re about to transition to. That was one of the reasons the founder chose Talkie: having everything in one place.
Talkie schedules into the practice’s existing schedule, tees up medication refills, and leaves messages for clinicians—all written back into the EHR.
Talkie handles scheduling, rescheduling, cancellations, refill requests, new-patient calls, and more.
Take just one slice of that—new appointments booked—and the picture is already striking.
In the roughly eight months since the agent went live, the Talkie agent has booked 800 appointments entirely on its own—100 a month—with no one at the front desk involved. A patient calls, picks a slot, and gets booked, start to finish.
A follow-up visit at the practice is ~$125, which works out to roughly $12,500 a month in booked-visit revenue tied to appointments the AI handled end-to-end.
Set against the cost of their Talkie subscription—and remembering this is just a fraction of what Talkie does for the practice—the return on AI scheduling is more than 10x.
And that is scheduling alone, before the reschedules, cancellations, refills, and new-patient calls Talkie also handles.
Additionally, the cost per call is a fraction of what the same coverage would cost in staff hours or an answering service, and Talkie covers nights and weekends a single hire never could.
What would the founder say to a practice still weighing whether AI belongs at the front desk? This is where society is heading, so practices have to learn how to use it, leverage it, and get comfortable with it. Other business sectors are already using this kind of technology—so why not bring it into healthcare, for the benefit of both the practice and its patients? He’d encourage anyone on the fence to give it a try and use it to take better care of patients.
That last point is a big part of why he brought Jessica on. For a medical practice, the phone is often the first encounter a patient has—and making sure every one of those calls gets a consistent, high standard of care, including the ones that come in after the clinic closes, is exactly the job he handed to Talkie.
